CMake definitions and format rearrangement 
- Rename some SFIZZ_ prefixed definitions where possible to permit some modules
  to be generic and be reused in other projects, keep project' information
  in the same place and more explicit
- Fixed non-compliant code formatting

c82d891e22 Add section suffix to man pages 
`man`, at least on some linux distros, ignores files
that don't have have the numeric suffix that corresponds
to the man page section.
This commit adds the suffix to the tools man pages.

7f352170a6 Logging is not implicitely handled by the synth 
You need a call to get the callback breakdown each frame.
sfizz_render has been updated to make use of this.

- Add the aftertouch modulation source
- Dispatch aftertouch events and track the aftertouch status in the midi state
- Parse cutoff_chanaft
- Convert aftertouch messages to an extended CC message
